Physician Finder (U.S.): Physicians who perform the NovaSure endometrial ablation, MyoSure and/or Acessa procedures have the opportunity to be listed on Hologic’s Physician Finder.

The listing is complimentary and can help patients get connected to a physician in their area. All names and contact information will be posted exactly as entered. If there are multiple names per practice, a listing is needed for each.

This is an internal form that can only be completed by Hologic employees due to required permissions; however, please confirm with your customer that they want to be featured on the physician finder before submitting this form.
